Open Doors (MFP)

Open Doors Program assists individuals living in a nursing home or Intermediate Care Facility (ICF) in making decisions regarding home and community-based services. BILS has Transition Specialists who directly assist people in nursing homes to access the services they need to return to the community. BILS Open Doors staff meets with individuals and their families to provide information regarding services and support available in the community. To be eligible for the program, you must have be active medicaid recipient, transitioning from qualifying institution (i.e., hospital, rehab, skilled nursing facility or intermediate care facility), must have been in the qualifying institution for at least 90 days at the time of discharge, must transition to a qualified setting (i.e., a home or apartment in the community owned or rented by the participant or their family members OR a group home with 4 or fewer unrelated individuals living together)

Open Doors Peer Support Program:

The Peer support program is designed to offer people who are considering or in the process of transitioning one-on-one support through the process by individuals who have lived the experience. Our Peer Advocates visit nursing homes and developmental centers to share their experiences with residents and support people who want to move to the community.
